Trigger bar spring jumping off

Hey all,
I've had my P64 for a while now, but have been running into a couple trouble spots.
I have had a problem with the safety. After I turned if off, the trigger would still pull all the way back as if it was still on. I saw another thread that gave me a couple of ideas.
My main problem is that the trigger return spring (the one that is set into a groove in the trigger bar) keeps coming off during firing. I know that it jumps off if the slide isn't on it (which I figured out during cleaning), but it's doing this while at the range.
Any ideas on solutions? Should I replace the spring? Is that bar replaceable if the groove is too warn?
If so, do you have any sources for replacement parts? I'd love to use this as a carry piece, but I can't rely on it unless I KNOW it'll fire if needed.
